Parent Verification of Student at-home COVID Results

Complete the form below to submit your student's at-home COVID 19 results.  Parents choosing this option are supporting the safety of all students by conducting this test to ensure their student's current quarantine exposure status.  Any student who test positive on a home test must not be sent to school in accordance with current safety procedures.

I understand that my verification is being used by the Calipatria USD as acceptable evidence for the current testing results of my student. I understand that my decision to utilize at-home tests is supporting the "Test to stay" opportunity for all students.
